Health Engagement

The Justlife Health Engagement Team are commissioned by Arch to work with a variety of people experiencing homelessness, who have specific needs. The team play a valuable role in enabling patients to obtain and maintain suitable accommodation, and access health and community services. They provide person-centred support to improve health, build independence, and transition into a secure home.

They support people to meet their necessary health needs and reduce the likelihood of hospital readmission. This could involve:

• Accompanying people from hospital to their accommodation
• Making sure they can access their prescriptions and health appointments
• Connecting them to mental health services
• Advocating for their health needs
• Helping people understand and progress their housing options
